What the research actually says about women and strength training

Resistance training is one of the few interventions with consistent evidence across the female lifespan. Trials in women report gains in lean mass, muscular strength, and bone mineral density at the hip and spine, along with improvements in insulin sensitivity and resting blood pressure. What is far less consistent is how those trials were designed, and how often women were included at all.

What the trials measured

Most randomized trials in this area run eight to twenty-four weeks and measure surrogate outcomes: one-repetition maximum, lean mass by DXA, and fasting glucose or insulin. Programs that progressed load over time produced larger strength gains than fixed-load programs, and two to three sessions per week was enough to move most of those markers in previously untrained women.

What they did not measure

Few trials stratified results by menstrual cycle phase, hormonal contraceptive use, or menopausal status, so questions women actually ask are still open. Long-term outcomes such as fracture rates are rarely captured because study windows are short. Reading these papers well means separating what was demonstrated from what was assumed and carried forward.
